GIRLS Players
- Copy of Birth Certificate or Passport (NEW players only)
- Photo
- Copy of front and back of insurance card
- IYSA Medical Waiver (2 sheets)* PLEASE DATE FOR 6/1/2024 to be good for the year.
- IWSL Player Registration Form (U8-U19)
- US Club Soccer Player Registration Form (U13-U19)
- US Club Communicable Disease Waiver (U13-U19)
*Please note that forms must have parent signature at bottom. If you fill out these forms electronically, please make sure they are still signed.
Boys Paperwork

1) print required forms from links above and fill them out. You will not be able to complete registration without uploading the required player forms.
2) scan your completed forms and save to your computer (scanning is much preferred, if you are unable to scan you can take a photo)
